The Dream Team: Family Supports, L.L.C. will provide quality services to individuals and families receiving one of the following State of Indiana Medicaid Waivers:

  1. Family Supports Waiver (FSW)
  2. Community Integration Habilitation (CIH) Waiver
  3. Money Follows Person Waiver (MPF)

 


Family Supports Waiver (FSW)

The Family Supports Waiver (FSW) program provides Medicaid Home and Community-Based Services (HCBS) to participants of any age residing in a range of community settings. The participant may either live at home with family or in their own home. The FSW serves persons with an intellectual/developmental disability, intellectual disability or autism. Individuals must also have a substantial functional limitations. The Individual’s team develops an Individual Service Plan (ISP) using a Person-Centered Planning process guided by an Individual Support Team (IST). The IST is comprised of the participant, their case manager and anyone else of the participant’s choosing but typically family and/or friends. The participant with the Individual Support Team (IST) selects services based on individual needs, identifies service providers of their choice and develops a Plan of Care/Cost Comparison Budget (CCB) commonly known as the “budget”. The budget is subject to an annual waiver services cost cap of $16,545. The Dream Team: Family Supports, L.L.C. will accept Medicaid Waiver payment as payment in full and will never charge the family for any services. Your Indiana Waiver Case Manager will provide you with a list of all services available through your dedicated waiver and is your first source of contact when choosing waiver providers. The Follow residential related services are available through The Dream Team: Family Supports, L.L.C.

• Respite (all services)
• Participant Assistant Care (PAC)
• Community-based Habilitation -Individual (CHIO)
• Transportation


The Community Integration Habilitation Waiver (CIH)

In addition to the above residential-related services and supports provided by The Dream Team: Family Supports, L.L.C. the Community Integration Habilitation Waiver (CIH) Waiver offers the additional services of Residential Habilitation and Supports and Community Transition (CT). The following services will be provided in a positive team-driven atmosphere where professional staff persons are highly trained on individual needs as created by the waiver team and implemented according to the Individual Support Plan (ISP).

1. Residential Habilitation and Supports (2)
a. RH10 (35 or less hours per week),
b. RH20 (above 35 hours per week

2. Community Transition (CT)-for individuals transitioning into the community from a group home or other institutional-like setting.


The Individual’s team develops an Individual Service Plan (ISP) using a Person-Centered Planning process guided by an Individual Support Team (IST). The IST is comprised of the participant, their case manager and anyone else of the participant’s choosing but typically family and/or friends. The participant with the Individual Support Team (IST) selects services, identifies service providers of their choice and develops a Plan of Care/Cost Comparison Budget (CCB). The CCB is commonly known as “the budget”. The CCB created through the individual’s support team is needs based and presented to the State of Indiana for review. The CCB (budget) does not have a specific cap for the CIH waiver; however, services needs must be clearly identified and documentation must meet standards guidelines for a specific service to be approved. It is extremely important for an individual and families to maintain close contact with their case manager and support team throughout the planning processes. The case manager gathers all documentation from all team members that will be submitted to the state for a budget review and approval.

Residential Habilitation

Residential Habilitation and Support Services (RH10 or RH20) may provide up to a full day (24-hour basis) of services and/or supports which are designed to ensure the health, safety and welfare of the participant, and assist in the acquisition, improvement, and retention of skills necessary to support participants to live successfully in their own homes.
